Please note that a 24-hour notice is required if you are unable to keep a scheduled appointment, or a fee will be charged.

How do I schedule my appointment?
Appointments are scheduled by phone call only. Please call our office at (808) 628-2837, and our team will help you find a time that fits your schedule and answer any questions before your visit.
Hours: Monday, Tuesday, Thursday, Friday from 9-5. Closed weekends and on Wednesdays.
Please arrive 10-15 minutes early to fill out any forms.
How does insurance and payment work?
We work with many insurance plans and are happy to help you understand your benefits. Coverage can vary by plan, so we recommend calling the office before your appointment with any questions.

What should I bring for my first visit?
Please bring:
- Complete New Patient Forms ahead of time
- A photo ID
- Your dental insurance card (if you have one)
- A list of current medications (and any allergies)
